Possible cause of failure:
Spindle shaft has heavy damage to the taper, it has a crack at one of the coolant plunger holes.
Bearing failure from coolant contamination, bearings and oil drains are full of coolant sludge.
Customer supplied tail bearing and ferrule fittings
Spindle came in without the drawbar assembly, or shaft splash ring
Drive dogs are slightly worn.
Front cover coolant plunger balls are very worn.
Taper coolant plunger assemblys are worn out.
Clean, inspect overall dimensions.
Replace bearings, oil lines, and orings.
Weld, and machine the cracked taper.
Grind, chrome plate and finish grind the taper.
Rework worn drive dogs.
Replace front cover coolant plunger balls
Replace taper coolant plunger assemblies
Adjust preload, balance, assemble and break in. Final quality control inspection.
