Possible cause of failure:
Rear bearing failure.
Rear shaft nut labyrinth is damaged.
Rear bearing’s may have gotten fatigued from excessive belt tension.
Clean, inspect overall dimensions.
Machine the rear shaft nut labyrinth damage.
Adjust preload, balance, assemble and break in. Final quality control inspection.
