This spindle failed from contamination entering the bearings. It appears to have slowly seeped into the front of the spindle over several years. We discussed some methods to prevent this from happening with our customer. One of the things we discuss is when the spindle is hot from running you need to wipe the coolant from the front of the spindle. As the spindle cools down when you shut the machine down it will pull the coolant into the spindle much like soldering a copper fitting.
Possible cause of failure:
Bearings are contaminated.
Clean, inspect overall dimensions.
Adjust preload, balance, assemble and break in. Final quality control inspection.
