Possible cause of failure:
Rear shaft nut was loose.
Shaft taper is worn with 42 microns.
Shaft journals are slightly egged and tapered with 4-5 microns runout (front +3/+8TE, rear 0/+6E) Customer said it had .001″ to a .0015″ runout.
We will need a collet from customer for testing
Clean, inspect overall dimensions.
Grind, chrome plate and finish grind the taper, front and rear journals.
Replace bearings, orings, belt and seals.
Adjust preload, balance, assemble and break in. Final quality control inspection.
