One of the most common reasons of failure is bearing contamination. This spindle fell under that category. The grease was washed out of the bearings by coolant. The seal was also worn and the OD pilot had 20 microns runout. We ground plate ground the OD pilot. The bearings were replaced. The preload was adjusted, the spindle was balanced, assembled and broke in. We tested the spindle in our QC department before shipping back to the customer.
