Possible cause of failure:
O-ring for the stator broken and out of position.
Front bearings grease washed out.
Front shaft journal is out of ISO.
Front housing fretted and 3 to 5 microns over ISO.
Drawbar ball cam damaged from use.
Clean, inspect overall dimensions.
Replace bearings, seals, orings, screws and gripper balls.
Grind, chrome plate and finish grind the front housing.
Grind, chrome plate and finish grind the front shaft journal.
Replace rotary union.
Replace drawbar cam tip.
Adjust preload, balance, assemble and break in. Final quality control inspection.
