Possible cause of failure:
Actuator mounting housing was damage during shipment.
Taper is worn, chrome is chipping out.
Front bearings failed from wood contamination, and are fatigued from use.
Gripper fingers are worn.
Tach sensor was not functioning.
Clean, inspect overall dimensions.
Replace broken housing with a refurbished one.
Grind, plate and finish grind the shaft 30 taper.
Replace bearings, grippers, tach sensor, bellevilles, and o-rings.
Adjust preload, balance, assemble and break in. Final quality control inspection.
