Possible cause of failure:
Bearings are very contaminated and failing.
Front nut and cover are rusted and very worn.
The rear bearing is OEM specialty and is no longer available.
Front housing is out of round and tight.
Taper is worn and has 8 microns of run out.
Taper threads are nicked.
A few wire numbers are missing for the stator.
Clean, inspect overall dimensions.
Replace bearings. Manufacture new nut and cover.
Machine the housing and cover to accept the replacement bearing.
Kiss grind the housing.
Kiss grind taper.
Thread file taper, some nicks will still be visible.
Send field out to number wires.
Adjust preload, balance, assemble and break in. Final quality control inspection
