Possible cause of failure:
First and second bearings failed. The number two bearing spun on the shaft and in the housing.
Shield for the oil line was snapped off.
Coolant plungers need to be replaced.
Drive dogs are hit, need to be replaced.
Inner spacer was destroyed by the bearing.
Clean, inspect overall dimensions.
Replace bearings and rear bearing, shield for oil lines, splash ring, plunger assembly, drive dogs, ferrules, and brass tubing.
Grind, chrome plate and finish grind the shaft journal and taper.
Balance shaft after grind, chrome plate and finish grind shaft.
Grind, chrome plate and finish grind the housing.
Manufacture a new inner bearing spacer.
Adjust preload, balance, assemble and break in. Final quality control inspection.
